Medical Case Management (MCM)

Project VIDA offers client-focused services under the Medical Case Management program. These services aim to connect clients with essential healthcare, psychosocial support, and a variety of other relevant services. A crucial aspect of MCM involves efficiently coordinating and following up on medical treatments to ensure comprehensive care.

Intensive Case Management

Intensive Case Management, an integral part of Project VIDA, employs a multi-step approach to swiftly facilitate access to both medical and psychosocial services. This service is designed for clients who have either lost contact with care or struggled to adhere to their care plan. The goal is to provide prompt support and coordination to help clients re-engage with necessary services.

DRS Case Management

(Department of Rehabilitation Services Case Management) The DRS Case Management program is a client-centered, home-based service. This intensive program caters to individuals with HIV/AIDS and provides comprehensive support, including in-home care coordination, assistance with daily living, treatment adherence counseling, and facilitation of connections to essential core services.

Workforce Development and Technical Assistance

Beyond health services, Project VIDA is dedicated to empowering individuals to reach their fullest potential. Through workforce development and technical assistance, we provide valuable skills and support to enhance employability and open doors to new opportunities.

Non-Medical Case Management

Project VIDA extends Low-Touch Case Management services to address diverse client needs. This inclusive service welcomes walk-ins and strives to assist clients requiring support.

Behavioral Health Services

At Project VIDA, evidence-based therapeutic techniques are at the core of the Behavioral Health program. This initiative offers effective interventions for issues like anxiety, gender-based violence, depression, and more. Services include individual, group, couples, and family therapy to comprehensively address clients’ mental health needs.

Housing Navigation

Project VIDA’s Housing Navigation services equip clients with the knowledge and resources essential for achieving and sustaining stable, long-term housing. Through addressing obstacles and crises related to housing, this program empowers clients to overcome challenges. It’s important to note that Housing Navigation provides valuable housing information services but does not offer rental subsidies or emergency housing.

Medication Adherence Program (Conexión)

The Medication Adherence Program, known as Connexion, is dedicated to supporting individuals who are HIV+ or taking PrEP. This program aids participants in effectively managing their medication regimens, connecting with relevant programs and services, and providing an array of incentives to encourage adherence and engagement in care.

T4T Program

A support group founded by trans individuals for trans individuals. Our T4T group celebrates and understands the unique challenges and experiences trans people face, and we’re here to create a safe and empowering space where trans voices can flourish. Our mission is to foster connections, provide resources, and amplify the diverse narratives within the trans community.

Caminando Juntos

Join our Caminando Juntos program, a weekly Spanish-speaking support group thoughtfully designed for members of the LGBTQ+ HIV+ community. At Caminando Juntos, we believe in fostering a safe and welcoming space where individuals can come together to share experiences, build connections, and find strength in unity.

Empowerment Echo

Coming Soon tuesdays from 4:30-6PM is Empowerment Echo. A Support group for Lesbian and Bi-Sexual Women Against Abuse. This group focuses on empowering those affected by domestic violence, survivors, and support systems.

A.L.L.Y.

Affirming Learning Loving Youth Provides LGBTQ+ teens with a safe space to discuss topics related to coming out and finding themselves. This group will provide teens with emotional support and encouragement. Not only that, it will provide the youth with a space to learn and build confidence in order for them to love
themselves.

LGBTQ+ Teen Drop-In Center

Teens 14-18 are welcome to join us Mondays and Wednesdays from 3:30PM-5PM at our wellness center 3501 W 26th Street.
We provide a Safe Space for LGBTQ+ youth to drop-in, play board games, switch, makeup center, crafts, snacks, and more.
