Our focus is on participation within the Chicago Metropolitan Service Area and surrounding suburbs. To request PV participation at your organization event please use this form. <br \/>For more information, please call us at 773-277-2291.<\/p><p>Guidelines for requesting Project Vida\u2019s participation at your event:<\/p><ul><li>This form must be completed for all community events.<\/li><li>Requests must be made at least four (4) weeks before the event.<\/li><li>PV Health participation is based on staff\/volunteer availability. <em>PV attendance is not guaranteed.<\/em><\/li><li>Please allow up to five (5) business days for a response.
